Other reasons for being banned would include:
illegal content (e.g. child porn)
reported content (in order to avoid legal disputes, or to please the government, google will sometimes remove sites)
spamming
click fraud through adsense

No, sites are not automatically reevaluated in order to be added back into the Google index.
If you're site was banned, Google will probably not give you a reason. However, you can probably guess it. When you have fixed your site, you can
submit your site for reconsideration.
