As I said in the title, I need a developer to create an opt-in form for a wordpress site.
I was using a basic version of an opt-in form that I got from Icontact but I want something a little more professionally done.
What I am looking for

I don't need anything fancy right now but I want it to blend into my site. http://www.dormroomcash.com.
